Tattooing can also be done in combination with surgical nipple/areolar reconstruction and is performed after reconstruction is complete. Please keep in mind that tattooing is often not covered by insurance. You may ask your plastic surgeon about financial grants you may apply for to help cover the costs. [node:field_related_content:0:entity:field_add_content_block] | What Is Breast Reconstruction?A plastic surgeon recreates all or part of a breast that has been removed surgically. The goal of reconstruction is to recreate the volume and shape of a breast, so that in clothing no one would know you have had a mastectomy. You will discuss the best method for reconstruction with your plastic surgeon based on your body shape, past surgical history, current health, type of mastectomy, cancer treatment needs and personal preferences.
The diagram below demonstrates the different methods of breast reconstruction and how they relate to one another. Click on any of the boxes below to learn more. Implant Based ReconstructionImplant based reconstruction uses silicone or saline implants to reconstruct the breast mound A reconstructed breast that appears like a natural breast in clothing. . Based on the type of mastectomy you undergo, the quality of your tissues after your mastectomy and your need for radiation treatment, you may be eligible for one of two types of implant based reconstruction, direct to implant or tissue expander to implant reconstruction. Direct to Implant Reconstruction (DTI)In a select group of women, a long term implant can be placed at the same time as the mastectomy either in front of or behind the chest muscle (pectoralis muscle). Your surgeon will determine whether this method is appropriate for you.
Women undergoing DTI reconstruction will often need an additional revision procedure which might include fat grafting A surgical technique that uses liposuction to transfer fat from one part of the body to another. to soften the contour of the implant and create a more natural appearing breast mound A reconstructed breast that appears like a natural breast in clothing. .
The image below illustrates the two locations that an implant can be placed and how ADM is used in either case. Tissue Expander to Implant ReconstructionTissue expander to implant reconstruction involves two stages. Stage A step in the breast reconstruction process defined by a separate surgery. 1: tissue expanders are placed in front of or behind the pectoralis muscle at the time of mastectomy or anytime afterwards. The expanders are temporary and are inflated over weeks to months to enlarge the breast space. Stage A step in the breast reconstruction process defined by a separate surgery. 2: tissue expanders are exchanged for a softer long term implant in an outpatient/same day surgery. Tissue Expansion ProcessDuring an expander fill, 50-60ml of sterile saline is injected into the expander through a port that lies just beneath the skin. The fill is usually painless. In cases where the expander is placed beneath the chest muscle (pectoralis muscle), you may experience mild to moderate soreness and muscle spasms after each fill. This can be treated with a prescription from your surgeon. Expander fills are done on a weekly or bi-weekly schedule until your goal fill is achieved. The endpoint of your expansion process depends on the final breast size you desire. For most patients, this will take approximately 6 to 8 weeks. Depending on the quality of your tissues during your mastectomy and expander placement surgery, your surgeon might decided to partially fill your expander in the operating room. This will decrease the number of clinic sessions needed to get to your goal fill. The video below demonstrates an expander fill session during a clinic visit. Tissue Based ReconstructionTissue based or flap reconstruction, uses tissues from another part of the body to recreate the breast. Tissue from the stomach, thigh or buttocks can be transferred to the chest. This is done using several different methods that can be divided into two groups, pedicled or free flap reconstruction. Pedicled Flap SurgeryIn pedicled flap surgery, tissue remains connected to its blood supply and is tunneled underneath the skin to reach the chest. The illustration below shows an example of pedicled flap surgery from the abdomen. This particular flap is called a TRAM flap (transverse rectus abdominis myocutaneous flap). A Pedicled TRAM Flap
Free Flap SurgeryIn free flap surgery, tissue is disconnected from its blood supply and reattached to blood vessels in the chest using special magnifying glasses or a microscope. Tissue from the abdomen, thigh or buttocks can be used.
Free Flap Surgery from the AbdomenFlap/tissue based surgery from the abdomen leaves a scar around the belly button, along your lower abdomen, and on your chest. The scar on your belly is usually low enough to be hidden by underwear or bathing suit. Because skin and fat is removed, your belly will appear flatter after surgery.
A TRAM flap can be done using free flap surgery techniques as illustrated below. When this is done, it is called a muscle sparing free TRAM flap (msfTRAM flap). A msfTRAM flap

Free Flap Surgery from the Thigh or ButtocksThis particular type of tissue based reconstruction is used for patients who
The common flaps from the buttock are the iGAP and sGAP flaps. The TUG and PAP flaps are taken from the thigh. Your plastic surgeon can discuss these methods in more detail if you fall into this category. The illustration below demonstrates free flap reconstruction using tissue from the inner thigh. Combined Breast ReconstructionCombined breast reconstruction uses implants and tissue from another part of the body to recreate the breast mound A reconstructed breast that appears like a natural breast in clothing. . The most common type of combined breast reconstruction uses one of the muscles of the back called the latissimus dorsi muscle. The back muscle and a portion of skin and fat on top can be rotated onto the chest to create a breast mound A reconstructed breast that appears like a natural breast in clothing. . The muscle is rotated onto the chest through a tunnel in the armpit. This will create bulkiness under the arm that will decrease over time. An implant or tissue expander is placed underneath the lastissimus muscle because most women do not have enough fatty tissue on their back to recreate a sizeable breast. If a tissue expander is placed at the time of the muscle flap, a second surgical procedure will be needed to exchange the expander for a long term implant. The scar will often be placed at or above the level of your bra line so that it remains hidden. Use of this muscle will not affect your ability to continue your regular activities but a select group of women (competitive swimmers, tennis players, rock climbers) may notice some shoulder weakness.
